Premera will no longer offer Landmark’s Home-Based Chronic Care program after 2020. Instead, we’ll use our medical management resources for members who need additional support with complex health conditions.
We’ve notified members about the transition, including who to contact if they have questions or need help. Providers who have patients currently using the Landmark program will receive additional communication over the next few months regarding patient transition and care needs.
