Care Gap Member Outreach

  • October 21, 2021

In September, eligible members in Washington and Alaska received communications about clinical gaps in care, preventive care opportunities, or information related to chronic conditions. Here’s more information about each activity.

    Emergency Department Utilization Mailer

This mailing provided members who have visited the emergency department more than 2 times in the past year with information about lower cost, and sometimes more accessible alternatives.

    Diabetic HbA1c Care Gap Letter

The purpose of this letter was to educate and remind members with diabetes of the importance of regular HbA1c testing. Members were asked to schedule an appointment with their healthcare provider to complete the test or discuss other options based on their diabetes care plan. View the tip sheet for more resources.

    Well-Child Mailer and Growth Chart

The American Academy of Pediatrics (AAP) recommends that babies have at least 6 check-ups with their primary care provider by the age of 15 months. The AAP also recommends that children age 2 and older see their doctor annually. View the tip sheet for information about patient claims and tips for success.

The parents/guardians of eligible members received the following:

    • Growth chart for ages 1 to 3 months old
    • Informational mailer for 4 months to 11 years old

All mailings included relevant, age-appropriate information including:

    • Recommended vaccine schedule
    • Key milestones
    • Anticipatory guidance
    • How to find a primary care provider

    High Blood Pressure Mailer

Federal Employee Plan (FEP) members with a diagnosis of hypertension received a mailer about taking care of their hearts, including information about a free blood pressure monitor, the importance of regular visits, taking medication as prescribed, and making healthy lifestyle changes to improve blood pressure readings. View the tip sheet for details.
