The following Payment Policies were annually reviewed with changes:
Drug Assay Services/Urine Drug testing:
Revised the policy title to add “urine testing”; policy title is now “Drug Assay Services/Urine Drug Testing.” Deleted terminated codes as of December 31, 2019. Added new codes effective January 1, 2020.
Global Surgery:
Clarified the description for “global surgery” indicator flag “ZZZ”
Manipulation Services:
Correction made to remove reference to “acupuncture” and put in “chiropractic manipulation” and “osteopathic manipulation.”
Modifier RA:
Changes made to make the policy in alignment with the Medical Policy 1.01.526 on “Durable Medical Equipment Repair/Replacement (Excluding Wheelchairs)”
Preadmission Testing:
In the PURPOSE section, clarified the policy only pertains to “med/surg” admissions. In the EXCEPTION section, identified the hospitals, units, and services that are exempt from the policy criteria
