Our 2018 Medicare Advantage plan members will have new ID cards. Here are a few changes you'll need to know:
- The primary care provider name is on the back of the card, along with the name of the designated clinic and phone number for easy reference.
- If your patient's plan has added benefits (dental or vision), these will show on the bottom front of the card.
- Contact information is still on the back of the card. We have a new phone number for pharmacy and mental health/substance abuse.
