To replace your old contract with the current OIC approved version, review, sign, and return the entire contract document and Dental Services survey within 15 days from the date of receipt. The survey helps us gather information for specific services your practice may offer so we can enhance our online provider directory and help our members find the care they need. Review the survey, check the services you provide, and send it back with your contract so we can add this information to your online profile.
Once we receive and process your contract documents, you’ll receive a counter-signed copy for your records. This process may take up to 45 days to complete.
No. There’s no gap between the two contract documents. We’ll retire the old contract after we’ve processed the new version.
We recommend that you keep a signed copy of each with your contract files.
