The Premera HMO Core Plus plan launched January 1, 2023. The HMO provider resource page has more information available.
HMO Resources:
- HMO Plus Team Customer Service: Designated for HMO only and can answer provider questions about the HMO Core Plus plan:
HMO: 844-PBC-HMO1 or 844-722-4661
- ID Cards: The HMO ID card serves as a member’s health plan identification and has unique HMO plan features listed.
- PCP Selection and PCP Find Care tool: HMO members and their dependents can choose an in-network primary care provider (PCP) using the PCP Selection Tool. They can use the PCP Selection Tool to change their PCP at any time.
- Availity: Availity is a single-source platform for the HMO health plan that allows providers to check member eligibility and benefits; monitor claim statuses and submissions; submit referrals and prior authorizations.
- PCP Roster tool: The PCP Roster tool allows providers to see the list of patients who selected them as their PCP or were offered to the member.
