Confirm Your Address for eviCore Requests

    You might regularly receive authorizations for care from eviCore, Premera's partner for outpatient rehabilitation services review. When your address is incorrect on an eviCore authorization, it can delay the payment of your claim. If your address is incorrect on the authorizations, here's what to do.

    First, confirm that your address is current in Premera's provider directory. Check our provider directory to make sure your address and phone number are accurate.

    If your address or other information has changed, fill out our provider update form and email or fax it to us. Updates to our directory can take up to 30 business days.

    If your address is already correct in Premera's provider directory but not on eviCore's authorizations, contact eviCore directly to correct your address on authorizations. You can reach eviCore's dedicated number for Premera providers at 800-792-8751 from 7 a.m. - 7 p.m., Monday through Friday.
