    For 2019 plans, select your PersonalCare Partner System from the following list (they're all part of the PersonalCare Partner Systems Network). You can also search the Heritage Signature* referral network used with these Partner Systems.

    *Important Notice for PersonalCare Plan Members: Heritage Signature is the referral network used with PersonalCare Partner Systems. Before seeing a provider in the Heritage Signature network, you MUST first choose a primary care provider and get a referral from your Partner System or you will be responsible for the full cost of the services you receive. Some exceptions to the referral requirement include: urgent care, acupuncture, spinal manipulations, maternity, mental health and substance abuse treatment. Please see your benefit booklet for more information.
