Incorrect prefixes were printed on some ID cards for 2020 Individual plans. See the list of prefixes by group ID numbers:
Claims: If you’ve already submitted a claim with an incorrect prefix, or submit one in the future, the claim will process.
New ID cards: We mailed new ID cards with the corrected prefixes to impacted members on January 31, 2020. Members may continue to use their initial ID card with the incorrect prefixes. Please be alert to this issue and use the correct prefix when you submit claims to Premera so that the correct prefix shows in your patients’ records.
