Premera HMO must be Notified of a PCP Change

    There are many scenarios when a member needs to change their designated primary care provider (PCP) to someone else, to include and not limited to:

    1. A member found another HMO primary care provider (PCP) that is a better fit for them.
    2. A member’s designated PCP is leaving the practice
    3. A member’s designated PCP is retiring or going on an extended leave.
    4. A member’s designated PCP is no longer carrying a panel.

    Premera HMO must be notified when a member changes their PCP to ensure claims are being processed accurately. Ask your patients to log into their account on Premera Blue Cross HMO or to call the HMO Plus team at the number on their ID card to designate a new PCP.
