For services on
or after January 7, 2022, prior authorization will be required for a non-malignant,
non-gender affirming hysterectomy. In addition, site of service review will be
part of the prior authorization for laparoscopic vaginal and vaginal hysterectomy. These policy criteria are aligned with those
from the American Congress of Obstetrics and Gynecology.
