Physician and Provider Relations Phone Line Is Closed

    The Physician and Provider Relations phone lines are currently closed. This closure will help us get caught up on provider requests, in order of receipt.

    Use online resources

    • Use our contracted provider update form to add a new provider, change contact information, clinic location, specialty, or tax ID number (W-9 required).
    • For questions related to credentialing applications (initial or recredentialing), send an email to Credentialing.Updates@Premera.com.
    • To join the Premera network (Washington and Alaska providers only), your organization must be contracted with Premera to be credentialed with us.
