Reminder! Massage Therapist Licensing & Billing

    Since July 1, 2017, Alaska state law has required all practicing massage therapists be licensed. Massage therapists who work for a Premera-contracted provider must also be individually credentialed and added under the contract.

    We no longer allow claims for services by licensed massage therapists to be billed under the supervising provider’s name. Services performed by a licensed massage therapist must be billed under their own claim form and the name of the licensed massage therapist must be listed on the claim form (CMS-1500 field 31).

    If you’re an independent massage therapist and you’d like to contract with Premera, you’ll need to complete a credentialing application and request a contract. You’ll find the application under “Credentialing” on our forms page.
