The purpose of this initiative is to educate and remind our members of the importance of following their diabetes care plan, regular exercise, and of the important tests they may be due for. The call to action is to schedule an appointment with their
doctor to complete the test or discuss other options based on the member’s diabetes care plan.

Emergency Department Utilization Informational Mailer
The purpose of this mailing is to provide members who have visited the emergency room (ER) more than 2 times in the past year with information about lower cost, and sometimes more accessible, alternatives to the ER.
Colorectal Cancer Screening Care Gap Letter
The American Cancer Society recommends that individuals receive a colonoscopy once every 10 years, a flexible sigmoidoscopy once every 5 years, a FIT-DNA test once every 3 years, or an annual FIT test.
