Premera is partnering with providers to improve clinical care across targeted measures for the Healthcare Effectiveness Data Information Set (HEDIS).
Use the posted 2017 HEDIS documentation and coding guide as a reference for Premera's targeted HEDIS measures. They are marked with an asterisk (*) within the guide.
While some targeted measures can be captured through claims data, nine are referred to as hybrid measures.
Hybrid measures require a review of the diagnosis and procedure codes that you submit on claims. For documentation that can't be submitted on a claim, a review of your organization's medical records may also be needed.
