Update Your Provider Directory Info

    Be sure to email us a completed provider update form if you have any of the following changes:

    • Adding/deleting a provider status at any of your practice locations
    • Changing your billing, practice, or remittance address
    • Adding a practice location or email address
    • Updating your tax identification number

    Help our members find you

    Make sure your information is also current in our provider directory. It’s important that providers are only listed at locations where they accept appointments. The Centers for Medicare and Medicaid Services (CMS) may call your practice to confirm that you’re following this requirement and verify that your information is current in the directory.
