| Where do I find the benefit information for bariatric surgery? |
Select Surgical under the Benefit/ Service Type when running an eligibility and benefits transaction. Bariatric surgery is a benefit listed under the Surgical Benefit/Service Type. Click the blue circle
icon following the Benefit/Service Type to see all the different services listed.

| When does the deductible apply to a member’s benefit? |
When doing an eligibility and benefits search for a Premera member, the deductible applies to all the member’s benefits unless otherwise indicated. See screenshot example below.

| Where is the benefit information for physical, massage, or occupational therapy? |
Select Massage Therapy, Occupational Therapy, or Physical Therapy under the Benefit/ Service Type when running an eligibility and benefits transaction.

| When doing an eligibility and benefits search, how do I select a member from the search results that display? |
Click on one of the member lines that displays from the search results before you click the submit button to run a transaction.
