Tool for Documenting Diabetic Retinal Eye Exam Results

    Premera encourages members with diabetes to have their annual diabetic retinal eye exam. However, the results of those exams aren’t always documented in the patient’s medical record for review and follow up by their primary care provider (PCP).

    While some large provider organizations have electronic health record (EHR) systems, diabetic retinal eye exams are often performed by independent eye care providers that use separate EHRs, which may make it difficult for providers to share information.

    To help get this important information into a patient’s medical record, we’ve created a form for providers to give their patients to take to their eye exam. The form can either be faxed back to the PCP’s office by the eye care provider or the patient can bring the document to their PCP at their next appointment. The form is also available in Spanish.
