    Did you know you can update your information in our provider directory to help patients find you? If your Premera directory information isn’t updated regularly, you could lose existing and potential patients.

    Per your contract agreement, you’re required to notify us as soon as possible when any of your office information has changed. Use this form to update your information and email the completed form to provider.relationswest@premera.com. Only list your practitioners and/or locations where they are accepting appointments.
