Handwritten Fax Forms No Longer Accepted Starting June 1

    Starting June 1, we’ll only accept electronic (not handwritten) prior authorization fax forms. Handwritten forms are often difficult to read, which significantly delays the processing time. For faster service, please submit your form by signing in to our online prior authorization tool or download and fill out the form online and fax it to us.  We recommend you check codes online to confirm a review is required before submitting a prior authorization request. This helps ensure timely processing of all qualifying requests.

    Use our online tools for the following requests:

    • Patient eligibility verification
    • Prior authorization code checks
    • Prior authorizations and status (if submitted by fax or online, for in-area providers only)

    A screenshot with the date can be used for verification if needed for an appeal.
