Credentialing TIN Requirement Update

  • May 6, 2021

Sometimes we receive W-9s that are incorrectly filled out or information is missing. Requesting more information can delay the credentialing process up to 30 days.

To make sure your credentialing is processed as quickly as possible, Premera is updating the tax information we request from providers.  Email a copy of your official SS-4 IRS confirmation letter (instead of a W-9) which includes your Employer Identification Number or EIN. This change also applies if you need to notify us about a change to your taxpayer identification number (TIN).

This change eliminates that waiting period and mirrors CMS requirements. Learn more about our credentialing requirements.
