Care Gap Member Outreach

    Some members in Washington and Alaska may receive the following communications from Premera in May:

    • Member scorecard or cancer screening care gap letter
    • Diabetes informational mailing
    • Asthma informational mailing
    • Preventive care emails

    There are more details below on each communication.

    Care Gap Letters or Member Scorecard

    Federal Employee Plan (FEP) members will receive either:

    1. A letter about the cancer screening they’re due for, if due for only one of the following:
      • Breast cancer screening
      • Cervical cancer screening
      • Colorectal cancer screening
      OR
    2. A scorecard if they’re due for 2 or more of the below screenings/test:
      • Diabetic HbA1c testing
      • Breast cancer screening
      • Cervical cancer screening
      • Colorectal cancer screening

    If members have not received a screening or test, more information about it will be noted. Likewise, if members have received a screening or test, a quick “great job!” message will be printed.

    Diabetes Informational Mailer

    Members with a diagnosis of diabetes should receive the following annually:

    • Hemoglobin A1C test
    • Cholesterol and triglycerides test
    • Retinal eye exam
    • Urine test

    The purpose of this mailing is to inform members of the importance of following their diabetes care plan, regular exercise, and of the important tests they may be due for. Members are encouraged to schedule an appointment with their doctor to complete the test or discuss other options based on their diabetes care plan.

    Asthma Informational Mailing

    Federal Employee Plan members with a diagnosis of persistent asthma who have been prescribed medication for their condition will receive an informational mailer about how to control their asthma. This mailing encourages members to take medications as prescribed, among other tips including avoiding triggers, getting a flu shot, scheduling regular visits with a provider, exercising safely, and developing an asthma treatment plan.

    Preventive Care Emails

    This email is intended to serve as a reminder to schedule a preventive examination and has resources to select and connect with a provider of their choosing. It’s sent to members who:

    • Have chronic or complex conditions and are due for a preventive examination
    • Are due for 2 or more of the following and are due for a preventive examination:
