Premera will partner with Home Access Health (Home Access) to offer preventive and disease monitoring screenings to members who are due for one or more of three tests. On behalf of the member’s primary care providers (PCP), Home Access will order fecal immunochemical tests (FIT), urine microalbumin tests, and hemoglobin A1C tests. In September, they will begin mailing the appropriate kit(s) to members to complete at their convenience.
The ordering provider is typically responsible for acting on the results of these tests, but for this program we’re asking the member’s PCP to follow up. If your patients receive testing, we’ll mail test results to you for review and managing next steps with your patients.
