Coverage Changes for Lorzone and Generic Chlorzoxazone Tablets

    Starting July 1, 2019, a pre-approval will be needed for brand Lorzone 375 mg and 750 mg tablets and generic chlorzoxazone 250 mg, 375 mg, and 750 mg tablets. Read the complete policy.

    If there’s a medical reason for a patient to remain on their current medication, you have two options:

    1. Submit documentation showing there’s a clinical reason why the patient isn’t able to use chlorzoxazone 500 mg tablets.
    2. Call our pharmacy services team at 888-261-1756 for a prescription review and approval. We’re available 7 days a week and holidays.

    Chlorzoxazone 500 mg tablets are significantly less expensive than brand Lorzone 375 mg and 750 mg tablets and generic chlorzoxazone 250 mg, 375 mg, and 750 mg tablets. Chlorzoxazone 500 mg tablets are scored and can be cut in half to deliver a 250 mg or a 750 mg dose, if needed.

    We know paying for high-cost medications is a concern for our customers. We regularly review the cost of prescription medications to determine if there are similar, but less expensive medications on the market. This policy change is an effort to help our customers find and use affordable alternatives to high-cost medications.
