Premera will partner with a third-party vendor to conduct the Be Healthy Nurse Outreach campaign for Medicare Advantage members with four or five gaps in care by late August. Nurses will discuss the following recommended preventive and other health services and offer to assist members with scheduling appointments:
- Mammogram
- Colorectal cancer screening
- Diabetic retinopathy screening exam
- Hemoglobin A1C test
- Kidney disease monitoring test
Nurses will also educate members about the risk of falls, the benefits of physical activity, and how incontinence can be treated.
