HEDIS Requests Start in February

    Premera Blue Cross Medicare Advantage plan will be collecting information for the 2017 Healthcare Effectiveness Data and Information Set (HEDIS®) from February through April 2017. Acting on behalf of Premera, Providence Health Plan may contact your office to request medical records for some of our Medicare Advantage members.

    We'll review office medical records to gather data on the following measures:

    • Comprehensive diabetes care, including eye exams
    • Hypertension control
    • Colorectal cancer screening
    • Adult BMI assessment
    • Medication reconciliation post-discharge

    If you're a primary care provider, endocrinologist, or eye care specialist for a Premera Blue Cross Medicare Advantage plan member whose records are selected for review, someone from our quality department will contact you to schedule a review or request copies of the records.
