Starting January 1, 2020, Premera is offering a new network for our Individual market health plans called Individual Signature. We’re no longer offering PersonalCare Plans. Instead, customers can select EPO plans that give them access to all providers in the Individual Signature network without a referral or waiver.
Below are images of the new member ID cards for the Individual market which will help you identify these patients.
Individual member ID card:
- Network is Individual Signature
- Individual Plan is marked in the bottom right quarter of the card
- Express Scripts is the pharmacy benefit manager
- There’s a new customer service number and claims mailing address on the back of the card
Grandfathered plan ID card:
- There are a handful of members with grandfathered Individual health plans
- Members with these plans have access to the Premera Heritage network
Dental ID cards:
- Individual health plan members have access to the Dental Select network
