    You are eligible to participate in the Health Connect Plan if you live in King or Snohomish counties. It’s important that eligible employees and their family members reside near the Health Connect Plan facilities and providers, which are located primarily on the Eastside.

    The plan includes two networks:

    • The Health Connect Network
      • Eastside Health Network
      • Allegro Pediatrics
      • Living Well Health Center
    • Extended network includes other Premera providers and gives you access to certain hospitals, including Seattle Children’s.

    The Health Connect Plan offers a dedicated customer service center to answer your questions and help you find a provider. Representatives are available to help you 5:00 a.m. to 8:00 p.m. Monday through Friday.
