    Health Connect Navigator

    The Health Connect Plan offers a dedicated service center called the Health Connect Navigator. Call (800) 676-1411 and select Health Connect Navigator for help with any of the following:

    • Choosing a PCP
    • Finding a specialist
    • Locating a clinic, urgent care center, or hospital
    • Scheduling appointments

    Representatives are available to help you Monday through Friday, 7:00 AM–7:00 PM Pacific Time.

    24-Hour Nurse Line: Health information 24/7
