To get the most out of the Health Connect Plan, you are encouraged to choose a PCP from the Health Connect network. Selecting a PCP in the Health Connect network when you enroll will make it easier for you to get
care later. If you don’t select a PCP, we will pick one for you and your enrolled family members, which of course you can change at any time.
Some services might not be available in the Health Connect network. When unavailable, you can receive them from an Extended network provider at the in network rate. That means lower out-of-pocket costs to you. Examples include:
