    The Health Connect Plan covers a wide range of primary care providers and specialists, but before you enroll, be sure to confirm that your and your family’s preferred providers are in the Health Connect network or the Extended network (which may be subject to additional out-of-pocket costs).

    The benefits of coordinated care

    Under the Health Connect Plan, your PCP gets to know your health care needs and goals and works closely with your other network providers—including sharing test results and medical records. Your PCP also works with a care team to help coordinate specialist visits, treatment plans, lab work and other tests, and medication management. That makes it easier for you to manage your health. It can also reduce duplicate tests and services, unnecessary treatment, and avoidable health care costs over time.
