  • Where to submit claims, prior authorization/pre-service requests, and more

    In Washington, Premera Blue Cross and Regence BlueShield share responsibility for claims processing, customer service, and prior authorization requests based on where the patient receives the service and what kind of service was received. Please follow the instructions to determine where to send your claims or correspondence.

    Locate the correct mailing address

    1. Determine the county where the service was performed* (see exceptions below)
    2. Select the area of the map that matches the color of the county where the service was performed. 
    3. Find the correct address listed under the type of service received (see explanation)

    Type of service definitions

    Professional Services are performed by a doctor or other healthcare professional. Examples: medical doctor, osteopath, optometrist, chiropractor, physical therapist, audiologist, independent laboratory, nurse practitioner, durable medical equipment supplier, ambulance services, applied behavior analysis services (ABA) or clinical social worker.

    Facility Services are services you receive for the use of a facility such as a hospital, emergency room, freestanding ambulatory facility, attended sleep study at outpatient part of a hospital, alcohol treatment facility or residential treatment center. Other specialized services (home hospice, organ/tissue transplants, and clinical trials).

  • *Exceptions

    Ambulance Claims

    Send your claim or correspondence to the Blue Cross Blue Shield plan in the state where the ambulance picked up the patient.
    Example: An ambulance picks up the patient in Oregon and transports her to a care facility in Washington. The ambulance claim will be processed by Blue Cross Blue Shield of Oregon. Consult the FEP local plan directory to find addresses and telephone numbers for other plans.
    Note: If the pick-up location is in Washington, refer to the Washington claims submission map to determine where to send your claim for processing.

    Durable Medical Equipment (DME)

    Send your claim or correspondence to the Blue Cross Blue Shield plan in the state from which the durable medical equipment was sent.
    Example: You live in Seattle and order parts for a wheelchair from a company in Montana. Blue Cross and Blue Shield of Montana will process the claim. Consult the FEP local plan directory to find addresses and telephone numbers for other plans.
    Note: If the equipment was sent from a DME provider in Washington, refer to the submission map to determine where to send your claim for processing.

    Laboratory Services

    Send your claim or correspondence to the Blue Cross Blue Shield plan in the state where the laboratory work was sent for testing.
    Example: The lab service you used is in Arizona but the sample was sent to Nevada for analysis. Blue Cross and Blue Shield of Nevada will process the claim. Consult the FEP local plan directory to find addresses and telephone numbers for other plans.
    Note: If the laboratory work was sent to Washington for testing or analysis, refer to the Washington claims submission map to determine where to send your claim for processing.
