- Scope of appointment forms are included in every kit.
- Sales brochures are region-specific and must be ordered separately to put in your enrollment kits.
If you work with an FMO, they have placed initial orders for kits and will receive them the last week of September. Please contact your FMO or fill out the 2021 Medicare Advantage producer supply order form to get your kits.
