Adult Dental Plans for Small Business

  • Every small business medical plan already includes an embedded pediatric dental plan. In addition, Premera offers separate dental plans for small business employees who are 19 and older. These can be paired with our medical plans to provide a broad range of covered dental services

    Family Dental plan

    • $150 Deductible
    • Diagnostic/preventive covered in full
    • For basic services you pay 100% until deductible is met, then 20% of the cost (coinsurance)
    • For major services that include crowns: you pay 100% until deductible is met, then 50% of the cost (coinsurance)

    Includes adult dental benefits that can be combined with the required pediatric dental benefits under the medical plan.

    Dental Optima plans

    • $50 Deductible
    • Diagnostic/preventive covered in full
    • For basic services you pay 100% until deductible is met, then 20% of the cost (coinsurance)
    • For major services: you pay 100% until deductible is met, then 50% of the cost (coinsurance)

    Optima
    Comprehensive coverage and flexibility in choice of dental providers.

    Optima Enhanced
    Better protection from surprise costs when using out-of-network dental providers.

    Optima Enhanced+
    Diagnostic and preventive services don't count against the annual maximum, allowing more money to go toward basic and major service coverage.

    • $50 Deductible for individuals/ $150 Deductible for a family
    • Diagnostic/preventive covered in full
    • For basic services you pay 100% until deductible is met, then 30% of the cost (coinsurance)
    • For major services: you pay 100% until deductible is met, then 50% of the cost (coinsurance)

    The Adult Dental Optima Voluntary plan, specifically for employers with 5 employees or more, provides dental coverage at little or no cost.
