    The 2019 plan changes are as follows:

    NEW Spinal Surgery Benefit at Select Blue Distinction Centers + (BDC+)

    Beginning Jan. 1, 2019, new spinal surgery benefit applies when surgery is performed at a Select BDC+ facility. These facilities have met national measures for quality and cost-efficient care.

    Benefits to having spinal surgery at a Select BDC+ facility:

    • There is no out-of-pocket cost to you, once you have met your Premera annual deductible
    • Travel benefits for you and a travel companion, including airfare, hotel, and transportation to/from the facility
    • Concierge and support services to make travel arrangements, transfer medical records, and schedule local post-operative care appointments

    The Select BDC+ facilities are located in:

    Alabama: Jackson Hospital, Montgomery, AL

    Alabama: Mobile Infirmary Medical Center, Mobile, AL

    Arizona: Tucson Medical Center, Tucson, AZ

    Arkansas: St. Bernard’s Medical Center, Jonesboro, AR

    Illinois: Loyola University Medical Center, Maywood, IL

    Mississippi: North Mississippi Medical Center, Tupelo, MS

    North Carolina: Duke Raleigh Hospital, Raleigh, NC

    Oregon: Sacred Heart Medical Center, Springfield, OR

    Washington: Valley Medical Center, Kent, WA

    The Select BDC+ Spinal Surgery Benefit includes:

    • Waived co-insurance (once deductible is met)
    • Travel benefits to and from the facility (50+ miles from the insured person’s residence):
      • Air travel to and from the facility
      • Black car service to and from the facility
      • Hotel accommodation for travel companion if more than 50 miles from insured person’s residence

    If you have spinal surgery at an in-network facility:

    • The plan pays 65% of the cost of the surgery and you will pay 35% (once deductible is met)
    • The travel benefit does not apply

    If you have spinal surgery at any other BDC+ facility:

    • The plan pays 85% of the cost of the surgery and you will pay 15% (once deductible is met)
    • The travel benefit does not apply

    Teladoc fee increase

    Beginning Jan. 1, 2019, the fee for a Teladoc medical consult will be $45 (or just $6.75 if you’ve met your Premera annual deductible).

    The fee for a dermatology consult will remain at $75 (or just $11.25 if you’ve met your Premera annual deductible).

    Medical Plus Plan Deductible will remain the same for 2019:

    Employee Only: $1,350
    Employee + one: $2,700
    Employee + 2 or more: $3,350

    IRS 2019 Health Savings Account (HSA) Contribution Limits

    The maximum HSA contribution amounts for 2019 are as follows:

    Individual (self-only coverage): $3,500
    Family coverage: $7,000

    This annual limit will include any pre-tax payroll contributions, post-tax contributions, or employer contribution, if any. If you will be age 55 or older during the plan year, you can contribute an additional $1,000 "catch up" contribution to the amounts above.

    Company HSA Contribution Amounts

    • Employee only: $300
    • Employee + one or more: $600
      • HSA contributions are paid biweekly
      • You must contribute a minimum of $2.00 per pay period
