    New Location and Tools for Health Management Program

    WEA Select Medical Plan enrollees now access the online health and wellness tools in the secure pages of the Premera web site. To access the program, enrollees will need to log in* to their Premera account and select the Stay Healthy link on the left-hand side of the home page.

    *First time users of the Premera secure site will need their Premera ID card to create a new account.

    The new tools include:

    • Health Assessment: Determine "health age" (Note: The engagement period will be held later this year; enrollees will be notified of details as we near the date.) 
    • Calorie Counter: Monitor calorie intake
    • Health Content and Symptom Checker: Research healthcare topics
    • Recommendations: Check out mobile apps and additional web site resources for more ways to fit wellness into a busy day.

    Affordable Care Act (ACA) Model Notice

    The ACA requires employers to provide each employee with a written notice containing the information regarding exchanges, and possible subsidies available when purchasing individual coverage through a public exchange. The Department of Labor (DOL) provides the document template. Districts will have to modify the template based on group or bargaining unit offerings. For more information, please go to the DOL site. As a point of information, the WEA plans meet the “minimum value” standard for 2013-14 coverage as referenced in Part B, Question 14, of the template.

    The WEA Select Wellness Programs are available 24/7 to subscribers on the WEA Select Medical Plans; their spouses; and covered dependents age 18 and over. The online suite of tools is designed to inform participants about health conditions, raise awareness of health habits that could be changed to enhance their well-being, and encourage them to think about their lifestyle choices.     Medicare Prescription Drug Creditable Coverage Notice All WEA Select medical plans include prescription drug benefits that meet "creditable" coverage criteria under Medicare guidelines. Employers are required to provide this notice to all employees hired between Nov. 1 and Dec. 31 of the calendar year who are Medicare-eligible or have dependents who are Medicare-eligible.
    Medicare Part D Notification Cover Sheet To help your employees understand why they are receiving the creditable coverage notice, you may choose to provide this sheet.
    Unum Life Change Beneficiary Form Employees should complete and return this form to their school district administrator as needed.
