    If your Premera directory information isn’t updated regularly, you could lose existing and potential patients. Premera is required to provide a complete and accurate provider directory to our members. Additionally, the Centers for Medicare and Medicaid Services (CMS) calls practices on a regular basis to confirm that information is current in the directory.

    Check your information in our Provider Directory by entering the practice or provider name in the search field on the page. Consider adding a reminder to your calendar to check your directory listing every 30 days, especially if you’re with a larger healthcare organization.

    If provider or practice information is missing or incorrect, use this form to update your information. Email the completed form to Provider.RelationsWest@premera.com or fax to 425-918-4937 and we’ll update your information. As you review your information, pay attention that your practitioners are only listed at locations where they accept appointments.
