    Our Provider Customer Service and Administrative Support teams are currently seeing a daily increase in provider faxes. The most commonly faxed items are related to claims (specifically BlueCard claims) and medical records.

    Please be aware that Provider Customer Service and Administrative Support teams are no longer accepting faxed correspondence.

    Here’s where to fax or mail the most commonly sent information:

    Prior authorization and inpatient admission

    Fax inpatient admission notification forms and related medical records to 888-742-1487.
    Fax prior authorization request forms to 800-843-1114.
