To improve health outcomes and simplify diabetes management for our members, Premera is expanding its diabetes management program. Beginning in 2020, Premera members may qualify for enrollment into the program supported by the following two healthcare partners:
- Livongo
- StepIn and LifeScan powered by Express Scripts Inc.
Livongo and LifeScan offer a diabetes management program that includes a remote blood glucose monitoring for those with diabetes. In addition, Livongo and StepIn offer a weight management program for those at risk to develop diabetes, which includes an educational curriculum, peer support groups and individual coaching sessions.
Eligible members will be identified through a combination of group enrollment and member criteria based on medical and pharmacy claims. Outreach will then be made by the appropriate partner using multi-channel enrollment information and steps. Livongo, StepIn, and LifeScan are available at no cost to members.
