Premera Blue Cross Medicare Advantage plans offer Medication Therapy Management (MTM) program for eligible Premera Part D members managing multiple chronic conditions and medication therapies. For 2018, our MTM program will be administered by OutcomesMTM. OutcomesMTM is an independent company that provides medication therapy management on behalf of Premera.
MTM programs work with physicians and other prescribing providers to deliver the best medication therapy to patients and to coordinate their medication therapy across multiple practitioners. As always, you make the final decision about changes in drug therapy.
What to expect
The 2018 MTM program brings together community pharmacists, a telephone team, and other channels to assess and address potential drug-related problems. These MTM pharmacists work collaboratively with prescribers to ensure members are on a safe drug regimen and taking prescribed therapies as their doctor(s) intended.
If questions on your patients' drugs therapies arise, your office may receive a call from an MTM Pharmacist. You may also receive a faxed request to review a patient's prescriptions.
How you can support medication therapy management
To support medication safety and help reduce your patients' out-of-pocket prescription costs, we ask that you:
- Review and respond to pharmacist inquiries or recommendations to address medication-related concerns.
- Encourage your patients to talk with their pharmacist about their medications. Pharmacies often have tools and programs to support your patients in taking medications as prescribed.
- Tell patients to share their discharge medication list with their preferred pharmacy to ensure pharmacy records are updated and the patient has the right medications at home.
