We recently made changes to the eligibility criteria for Landmark. Because of these changes, some Medicare Advantage members who are currently receiving care from Landmark will transition to our Medicare Advantage Medical Management program. This transition will happen over the next six months.
What do you need to know?
- We’re notifying affected patients about the change.
- Landmark will reach out to these patients to help them with the transition.
- Landmark will contact the patient’s provider if they see a need to discuss care plans in detail.
