Premera is partnering with Matrix Medical Network (Matrix) to do in-home comprehensive health assessments (CHAs) and preventive screenings for Medicare Advantage members. These visits and screenings help members who have difficulty arranging an annual wellness visit with their primary care provider (PCP).
Preventive screenings
We have asked Matrix to order some preventive screenings for members who are due soon or overdue for these assessments. Matrix is ordering fecal immunochemical tests (FIT), urine microalbumin tests, and hemoglobin A1C testing on behalf of each member's PCP, if the member is due for that test.
Follow up by each member's PCP
The ordering provider (in this case, a Matrix provider) is typically responsible for acting on the results of these tests, but for this program, we're asking the member's PCP to follow up. If your patients receive testing, those results will be delivered to you for review and next steps.
