In-Home Health Assessments

    Medicare Advantage members will now have access to in-home health assessments and preventive screenings. These visits and screenings will help members who have difficulty arranging an annual wellness visit with their primary care provider (PCP). After each visit, a health assessment summary is mailed to the member’s PCP for review.

    Preventive screenings

    Premera will partner with Matrix Medical Network (Matrix) and Signify Health (Signify) to offer preventive screenings to members who are due for a screening. On behalf of the member’s PCP, Matrix and Signify will order fecal immunochemical tests (FIT), urine microalbumin tests, and, if the member is due, hemoglobin A1C tests. They’ll provide the appropriate kit to the member to complete at their convenience.

    Signify will also conduct a diabetic retinopathy screening exam during the assessment for members who are due for this service.

    Follow up by each member’s PCP

    The ordering provider is typically responsible for acting on the results of these tests, but for this program we’re asking the member’s PCP to follow up. If your patients receive testing, we’ll mail test results to you for review and managing next steps with your patient.
