On January 1, 2020, seven Providence St. Joseph Health facilities will begin service as Premera’s Designated Centers of Excellence for Total Joint Replacement (TJR).
Premera members seeking TJR must use a designated center of excellence (COE). If they don’t use a COE, they may be responsible for the total cost of care. Before submitting a prior authorization for a total joint replacement, ask your patient to contact customer service at the number listed on their ID card. This will guarantee that the member gets accurate information about their benefits and what facilities they may use for care.
