Prior Authorization
- For prior authorizations through December 31, 2019:
Check the Premera code list to see if prior auth is required. To submit a request, use our prior authorization request form. If you have questions, call 877-342-5258, option 3.
- For prior authorizations after January 1, 2020:
Prior authorization for 2020 Individual Plan members will be available using the Identifi tool link and our 2020 code list on the new secure Individual provider website starting on January 1, 2020.
