    2014 Employee Benefits

    Associates who work at least 20 hours per week are eligible for the following benefits. Eligibility generally begins the first of the month following the date of hire.

    Health Savings Account

    • Premera Blue Cross makes an annual contribution to this account. The contribution amount depends on the medical plan coverage level you enroll in
    • Associates have the option of making pre-tax contributions to the account in addition to the employer contribution

    From one to five times annual base salary, up to a maximum of $1,000,000

    Disability Insurance

    Short-Term Disability Plan Options 

    • 14-day waiting period, 60 percent of base salary up to $1,500 per week paid for by Premera
    • 14-day waiting period, 66 2/3 percent of base salary up to $1,500 per week

    Long-Term Disability Plan Options 

    • 90-day waiting period, 60 percent of base salary up to $15,000 per month paid for by Premera

    • Pay for out-of-pocket healthcare expenses with pre-tax dollars

    • Eligibility: Enrolled in the Health Savings PPO Plan
    • Pay for out-of-pocket healthcare expenses, after the Health Saving PPO Plan deductible is met, with pre-tax dollars

    • 22 Funds managed by Vanguard
    • Associates may contribute up to 50 percent of their earnings on either a pre-tax or Roth post-tax basis, subject to annual IRS limits
    • Premera makes a matching contribution of 50 percent on the first 6 percent of the amount contributed
    • Eligible to participate immediately upon hire
    • 100 percent vested after three years of service (from date of hire)

    Pension Equity Plan (PEP) 

    • 100 percent company-paid defined benefit
    • 100 percent vested after three years of service
    • Ability to select lump sum amount or monthly annuity upon retirement

    Paid Time Off

    Associates who work at least 20 hours per week are eligible for Paid Time Off (PTO), a combined pool of time that includes vacation, personal days and sick time.

    Full-time associates receive eight hours of pay for each day while part-time associates receive pro-rated pay based on scheduled work hours.

    Bereavement Leave 

    Up to three days per incident, with a maximum of five days granted due to extenuating circumstances.

    Family Medical Leave 

    Up to 12 weeks of unpaid leave time in a 12-month period to care for yourself, spouse, domestic partner, child, parent, or parent-in-law. Associates must meet the eligibility requirements as defined by state and/or federal laws.

    Personal Leave 

    Premera may allow a personal leave of up to 30 days in approved situations.

    Education and Development

    Educational Assistance Program 

    The Educational Assistance Program pays 100 percent of your tuition costs, lab fees and required course-related textbooks, up to a maximum of $2,000 each calendar year for full-time associates and $1,000 per calendar year for part-time associates. The maximum for courses towards a graduate degree is $5,000 each calendar year for full-time associates and $2,500 for part-time associates.
