The HEDIS® (Healthcare Effectiveness Data and Information Set) Plan All-Cause Readmissions measure addresses the critical issue of patients returning to the hospital after recent discharge, for avoidable reasons. These resources can help you keep your patients out of the hospital:
- Your Premera patients can call the Premera 24-NurseLine when your office is closed. A registered nurse can triage your patients to the best care option and possibly avoid an unnecessary readmission. The NurseLine phone number is on the back of your patient’s Premera ID card.
