HbA1c Testing for Patients with Diabetes

    Premera’s Quality Member Engagement Team is sending Federal Employee Program (FEP) members with diabetes a hemoglobin A1c test kit. Approximately 250 members in Alaska and eastern Washington who haven’t had the test this year and don’t have a Primary Care Physician will receive kits.

    The purpose of this initiative is to reach members that don’t already have a Primary Care Physician, but still need a hemoglobin A1c test. Members that participate in this program will receive their results within two weeks.

    Members who complete the test kit whose hemoglobin A1c result is 8% or greater will receive a call from a Premera Quality Nurse to discuss the results. The Nurse will also assist them in acquiring a primary care physician who can help them manage their diabetes.

    Provider offices may receive calls from members asking for an appointment or follow up after the test kit that was mailed to them.
