    The strength of our dental network and access to nationwide contracted dental providers makes it easy for employees to find the right dentist for their needs, and it translates into greater savings for in-network services.

    • With Premera DentalBlue plan, employees have access to one of the largest dental provider networks in Alaska
    • Providers in our network accept our allowable charges as payment in full, which can save members money. Providers not in our network may charge back to the member the difference between our allowable charge and their billed charge.

    Group Enrollments

    Completed implementation materials must be received at Premera well in advance of the first of the month to ensure eligibility and receipt of ID cards by the groups start date.

    Follow these steps to complete your enrollment:

      Circle, sign and date the Rate Exhibit to acknowledge your plan choice(s)

      Other Coverage Questionnaire

    2. Return completed enrollment documents to AlaskaRFPs@premera.com or your Premera sales professional
    3. Make a check for at least 90% of the first month's premium payable to Premera Blue Cross Blue Shield of Alaska and mail to:

    Group Renewals

    Completed renewal documents must be received at Premera well in advance of the first of the month for renewing groups beginning coverage on the first of the following month to ensure eligibility and receipt of ID cards, by the groups start date.

    2. Enrollment spreadsheet (membership adds/drops or plan changes)-Please contact your account management team to obtain this document
